A Letter from Dr. Rheam to her patients:
After practicing Optometry for 39 years in Mountain View, I have decided to retire! I would like to take this opportunity to thank you for your continued trust and confidence in allowing me to serve as your Optometrist throughout the years.
My official retirement date will be October 31st and I will no longer be seeing patients. I highly recommend a referral to my colleagues at Mountain View Optometry who will be happy to continue your optometric care. You can review the bios of our doctors on our website (mvo.com).
Your medical records are confidential and will remain on file with MVO. If you choose another Doctor within our organization, you will not need to take any action concerning your records. If you choose a Doctor outside of our clinic, you will need to sign an authorization form before we can release your medical records. Please note, by law we cannot share your medical information without your written consent. You may sign this form at our office or you may complete the necessary paperwork at your first visit with your new doctor.
It has been a great pleasure meeting and caring for you all. I sincerely appreciate your friendship and loyalty over the years. I wish you continued good health, great vision and all the best in the coming years.
